Job Description
Forefront Telecare is currently looking for PMHNPs to cover a SNF seeing 8-10 residents one time per month.
Must see patients in the morning. No evening/night shifts available.
Will work closely with SNF staff and a care coordinator.
There are 5 facilities total.
Requirements
- 2-3 years experience in practice as a Psychiatric APRN/PMHNP preferred
- Current license for the state of Pennsylvania
- Board Certified (PMHNP-BC) by ANCC
- Clean NPDB
- Multi-state licensed preferred
- TelePsych experience preferred
- Updated CAQH preferred
- Active DEA Controlled Substance Registration
- Friendly and compassionate disposition
- Effective communication skills
- Team player
- Knowledge of evidenced based interventions are required to care for patients of all ages who are experiencing different behavioral health crises and substance abuse.
- At least 1 year experience in Telehealth
- Experience with Geriatric patients
- Will be required to use Athena EMR
- Experience with medication management/GDR is a must
