Responsibilities

As a Chaplain/Bereavement Coordinator at Health Link Home Health and Hospice, you will play a crucial role in providing spiritual care services to patients and their families/caregivers. Your primary responsibilities will include


  1. Spiritual Care: Utilizing your theological education and expertise, you will deliver compassionate and culturally sensitive spiritual care to patients and their families/caregivers. This will involve offering support, counseling, and guidance during times of illness, grief, and loss.
  2. Bereavement Coordination: You will facilitate bereavement support services and coordinate grief counseling for families/caregivers after the loss of a loved one. By providing a compassionate presence and resources, you will assist individuals and groups in navigating their grief journey.
  3. Interdisciplinary Collaboration: Working as an integral part of the interdisciplinary team, you will collaborate with healthcare professionals to ensure holistic care for patients. Your ability to contribute to the team dynamic and integrate spiritual care into the overall treatment plan will be crucial.
  4. Quality Assessment and Performance Improvement: Actively participating in quality assessment and performance improvement activities, you will contribute to enhancing the overall care provided by Health Link Home Health and Hospice. Your insights and suggestions will help shape our services to meet the evolving needs of our patients and their families/caregivers.

Requirements

To excel in this role, the successful candidate will possess the following qualifications:


  • Certificate of graduation from an accredited seminary or school of theology (BD, MDiv, or equivalent theological degree) or appropriate certification in hospital or pastoral ministry.
  • Previous experience working with death and the dying, particularly in relation to patients and their families/caregivers, is preferred.
  • Ability to work collaboratively as a contributing member of an interdisciplinary group, ensuring seamless care coordination.
  • Comfort in an inter-faith setting, demonstrating respect and understanding for diverse spiritual beliefs and practices.
  • Active participation in quality assessment and performance improvement teams and activities.
  • Knowledge of and commitment to hospice care philosophy, with a deep understanding of death, grief, and bereavement.
  • Understanding of grief responses and experience in providing counseling to individuals and/or groups dealing with loss.
