As a compassionate and specialized Wound Care Nurse (RN), you will have the opportunity to positively impact the lives of our patients by providing wound care management services in the comfort of their own homes. In this role, you will be responsible for performing thorough assessments of patients with wounds, creating individualized care plans, and closely monitoring wound healing progress. Collaboration with interdisciplinary team members, such as physicians, physical therapists, occupational therapists, and other healthcare professionals, is essential to ensuring the best possible care for our patients.
Our ideal candidate is an experienced RN with a genuine passion for delivering high-quality care to patients in a home health setting. You should possess exceptional clinical assessment skills, with the ability to develop and implement personalized care plans for patients with wounds. Additionally, strong communication skills are critical, as you will be responsible for educating patients and their caregivers on wound care management and prevention.
- Perform comprehensive assessments of patients with wounds, including evaluation of the wound, underlying causes, and patient factors that may affect wound healing.
- Develop and implement individualized care plans for patients with wounds, including wound care and management, nutrition, pain management, and education on prevention of further skin breakdown.
- Monitor and document wound healing progress, including wound size and appearance, drainage, and pain management.
- Collaborate with interdisciplinary team members, including physicians, physical therapists, occupational therapists, and other healthcare professionals to coordinate care for patients.
- Educate patients and caregivers on proper wound care and prevention of further skin breakdown.
- Participate in performance improvement activities and quality assurance programs to ensure best practices are implemented for wound care management.
Requirements
- Current Registered Nurse (RN) license in good standing with the State Board of Nursing in California.
- At least 1 years of wound care experience, with demonstrated proficiency in wound assessment, treatment, and documentation.
- Strong clinical assessment skills, with the ability to develop and implement individualized care plans.
- Ability to work independently, as well as collaboratively with interdisciplinary team members.
- Excellent communication skills, with the ability to educate patients and caregivers on wound care and prevention.
- Experience with Kinnser electronic medical record systems preferred.
- Current Wound Care Nursing Certification (WCC) or Wound Ostomy Continence Nursing Certification (WOCN).
