The Client Care Manager is responsible for coordinating and monitoring the implementation of care plans to meet Client needs, collaborating with healthcare professionals and community partners, and providing ongoing support and advocacy. They investigate and resolve Client adverse events, conduct site visits to evaluate Client well-being, and facilitate all other monitoring. Additionally, they maintain accurate documentation of Client evaluations and events, ensuring clarity, completeness, and adherence to all Client related policies governing in-home care.

Requirements

Client Case Management:

  • Coordinate and monitor the implementation of care plans, ensuring Client needs are met.
  • Collaborate with healthcare professionals, social workers, and other community partners.
  • Provide ongoing support and advocacy for Clients, ensuring access to necessary resources.
  • Investigate, report and resolve Client adverse events, issues, complaints, changes, concerns, and requests.

Conduct Client Site Visits:

  • Conduct site visits between 7 and 30 days after the start of service.
  • Evaluate Client well-being, satisfaction, and adherence to the care plan.
  • Make necessary adjustments to the care plan based on evaluations.
  • Ensure timely completion of the in person 90-day monitoring process for all Clients.
  • Collaborate with the care team to address identified challenges or modifications.

Maintain Documentation:

  • Accurately record all documentation related to Client evaluations, monitoring, and adverse events.
  • Review and proofread documentation for clarity, completeness, and adherence to policies.
  • Submit finalized documentation to the Executive Director for final review.
  • Copy approved documentation into Sharepoint, Wellsky, and the Adverse Action Binder for future reference.
