ABOUT CITYSQUARE | NEIGHBOR HEALTH SUPPORT PROGRAM
CitySquare is a nonprofit based in Dallas, Texas that exists to fight the causes and effects of poverty through service, advocacy and friendship. For over thirty years, CitySquare has grown to be one of the most effective social services organizations in the city with multiple programs working together to address housing, hunger, health, and hope. Working hand in hand with our neighbors - we do not have clients - we develop sustainable programming and partnerships that provide support and encouragement so that all can achieve their full potential.
The Health Equity and Food Security department is made up of Assistant Directors, a Community Health Nurse, Program Managers, Community Health Workers, Resource and Health Navigators, a variety of Food Pantry Operational Staff, Child Nutrition Specialists and AmeriCorps members and/or interns to support the overall work of each program. The Health Equity department works closely in partnership with Parkland Hospital and other system level entities. The department operates under a larger strategic goal that every neighbor is connected to a healthcare provider and social supports. The food Security department consists of feeding programs under the Child Nutrition arm that includes Child and Adult Care Food Programs (CACFP) and Summer Food Service Programs (SFSP) and an onsite robust food pantry. The department operates under a larger strategic goal that every neighbor has accessible, affordable, and healthy food. Team members are passionate about working with neighbors through journey while also managing reporting requirements necessary to maintain grant funding, including accurate grant reporting and data entry. Team members effectively communicate through in-person meetings with fellow staff, interactions with neighbors, and communication with all stakeholders necessary to the vision of restoring hope for the thousands of neighbors living in poverty in our city.
POSITION SUMMARY
The Neighbor Resource Center (NRC) is a space dedicated for neighbors seeking information, referrals, and navigation for urgent and essential needs. The NRC is staffed with a Program manager, who provides supervision for resource navigators, interns and AmeriCorps members. The Resource Navigator works alongside neighbors experiencing or at risk of homelessness, battling economic insecurity, and/or need support with a variety of critical needs, to empower them in navigating and accessing community resources or internal CitySquare services. The Resource Navigator is an integral part of the team and serves as a trusted member of the community that they are serving. Resource Navigator works to build a trusting relationship with neighbors and serves as a liaison between CitySquare, the neighbor and partner agencies to facilitate access to services and improve the quality and cultural competence of service delivery.
The Resource Navigator will provide information, referrals and any necessary navigation services that assist neighbors, agencies and CitySquare staff who visit the center in person, via phone or e-mail. The Resource Navigator will be responsible for appropriate identification of neighbors to ensure they are served by the proper program. The Resource Navigator will conduct neighbor intake assessments, social risk assessments, assist in follow-up and promote self-sufficiency through social support and advocacy.
The Resource Navigator will be required to work onsite, document appropriate interactions in CitySquare database and answer daily phone calls and e-mails.
Requirements
QUALIFICATIONS
Education: Must have High School diploma or GED
Experience: in community outreach, sales, customer service or related work amp; working directly with vulnerable and homeless populations required; Three (3) years’ experience preferred.
Licenses/Certifications: Community Health Worker certification preferred, but not required
Additional Requirements: Proficient in Microsoft Office applications. Must have a valid driver’s license and insurance and reliable transportation. Must be able to work occasional over-time and flexible hours as requested. Must be able to travel to partner sites. Must be able to read and write English, fluency in Spanish is preferred but not required.
