Responsibilities
- The HCP of Texas Registered Nurse Case Manager is responsible for the admission, planning, intervention, and ongoing evaluation of patients in home care.
- RN Case Manager will partner with an LVN and have an assigned territory within a manageable proximity. Being a nurse isn’t a job, it’s a calling. Many of our highly skilled compassionate nurses live in the communities where they care for patients, providing much needed care to people in their community. Requirements
- Home Health (Geriatrics) experience
- Active RN License in the state of TX
- Knowledge of Wellsky (Kinnser)
- Driver's License and Vehicle with Current Auto Insurance
- Problem solving skills and ability to multi-task
- Excellent Communication Skills
- Positive, Professional attitude
