At Phoenix Home Care amp; Hospice, we invest in our nurses by providing a variety of pediatric vent, trach, g-button amp; g-tube training. As a Phoenix nurse, you will have access to continuing education opportunities through in-office training, while also receiving paid on-the-job specialized training with your special needs client. You will also receive 24/7 nurse support!
We are looking for a compassionate LPN or RN to provide pediatric home services for clients in Kansas City amp; surrounding areas.

Responsibilities
- Delivering compassionate, individualized nursing care to patients based on their unique needs and treatment plans.
- Assists in development, review amp; revision of the plan of care.
- Building strong emotional connections with patients and their families to offer a supportive and reassuring presence.
- Observes, records amp; report's reaction to treatment amp; any changes in client’s condition to appropriate personnel and/or physician.
- Provides ongoing instruction amp; supervision of client, family or other health team personnel in the plan of treatment.
- Monitor amp; record patient's condition amp; document provided care services via electronic charting.
- Participates in education programs, quality assessment activities, nurse’s meetings amp; other staff meetings as required.
Requirements
- Active Licensed Practical Nurse or Registered Nurse License
- Problem solving skills amp; ability to multi-task
- Excellent Communication Skills
