JOB SUMMARY

The Concierge functions independently to coordinate all necessary arrangements for clients admitted at the skilled nursing facility. The incumbent must assure that medical services are coordinated in a timely, accurate and courteous manner according to established guidelines. Will have proactive visits to newly admitted clients, ensuring any special needs have been met. The concierge does almost everything for clients that clinical staffs aren’t required to do to make the facility stay as comfortable and convenient as possible. The concierge consults with nurse managers and social workers to find out how they can meet the needs of newly admitted clients and those who stay for extended periods. The concierge organizes services to help clients, visitors and employees with daily needs that may otherwise be difficult to meet in a skilled nursing setting.

JOB RESPONSIBILITIES

  • Ensures that exceptional client experience is achieved by displaying effective customer service, problem solving and communication skills in a friendly, thoughtful and empathetic manner.
  • Performs a variety of clerical functions including managing clerical, coordination of data flow and sharing of general facility information or directions.
  • Consults with nursing supervisors, social workers, and the administrator to find out how they can meet the needs of newly admitted clients and of those who stay for extended periods.
  • Organizes services to help clients, and visitors with daily needs that may otherwise be difficult to meet in a facility setting. Create a friendly and cooperative atmosphere for visitors and facility staff members.
  • Schedules client meetings and appointments.
  • Acknowledges and greets all clients and visitors within five feet with a professional and friendly demeanor
  • Call clients by their appropriate titles and last names during interactions.
  • Answers phones using proper telephone etiquette
  • Provides clients with general information about the facility, services and area.
  • Work in conjunction with the Interdisciplinary Team (IDT), including Bed Control, Environmental Services, Food Services, Nurse Managers, on impending admissions and discharges and coordinate with the team the level of services to be provided.
  • Communicate with the Social Services Director and Case Manager on a daily basis to review the census of newly admitted clients and visit these clients documenting each client visited in the AOD Point of Care or Care Tracker tracking system, within 2 hours of seeing the client.
  • Communicate with nurses and CNAs daily to assist with any special needs they may know of from clients on their units and/or to let them know of any concerns the client may have that they are unable to resolve.
  • Consult with the Charge Nurse and/or Dietitian of any client requesting special food assistance to ensure the client may be granted their request.
  • Work with Environmental Services to ensure maintenance of the facility amenities, including room set-up, refrigerator, computers, DVD, toiletries, linens, bedspreads/blankets, etc.
  • Work with Food Services and routinely verify with client and family the quality of delivery of foods services, proper menu, and snack items.
  • Coordinate any special order goods as needed to meet culturally diverse populations, i.e. kosher, hallal, and vegetarian, etc.
  • Assure that special equipment are available, as needed, including wheelchair, cart, furniture, and other equipment necessary to meet the needs of clients.
  • Arrange for transportation for identified clients and families to and from the airport, appointments to and from surrounding hospital admission, hotel, and outlying clinics. Arrange for valet parking service and limousine service when client load demands additional transportation services.
  • Charged with ensuring all needs of the client and family are met throughout their visit. Effectively troubleshoots any problem independently and expeditiously.
