As a Home Health Physical Therapist, you will be responsible for conducting patient evaluations, developing therapeutic treatment plans, and providing personalized care to patients in their homes. • Conduct patient evaluations and develop personalized therapeutic treatment plans.
• Provide hands-on therapy to patients in their homes.
• Educate patients and their families on proper exercises and techniques to maximize physical function and prevent injury.
• Monitor patient progress and adjust treatment plans as needed.
• Collaborate with other healthcare professionals, including physicians and nurses, to deliver coordinated care.
• Maintain timely and thorough documentation of patient progress and treatment plans.
• Adhere to ethical and professional standards and ensure regulatory compliance.

Requirements

• Physical Therapy degree from an accredited institution.
• Current and valid state Physical Therapy license.
• At least one year of experience as a PT in a home health setting is preferred.
• CPR certification.
• Ability to work independently, prioritize tasks, and manage time effectively.
• Strong communication, interpersonal, and problem-solving skills.
• Comfortable with technology, including electronic medical records and mobile devices.
• Valid driver's license and reliable transportation.
