At 315 Health, we are dedicated to providing comprehensive healthcare services to our patients. We are currently seeking a highly skilled Registered Nurse (RN) to join our team as an Annual Wellness Visit (AWV) Nurse. As an AWV Nurse, you will play a vital role in helping our patients achieve and maintain their optimal health through preventive healthcare services. • Conduct virtual Health Risk Assessments before Annual Wellness Visits for Medicare patients
• Evaluate patients' health status and medical history
• Perform various assessments and screenings, including cognitive evaluations, depression screenings, and falls risk assessments
• Document and identify patient needs and identify areas for patient growth and improvement
• Manage and update patients' health records and documentation
• Make referrals to community support and resources
• Evaluate for and educate on Continuous Care Program eligibility
• Collaborate with healthcare teams to ensure coordinated and comprehensive care
• Stay up-to-date with the latest guidelines and regulations regarding Annual Wellness Visits

Requirements

• Active and unrestricted Georgia RN license (compact preferred)
• Minimum of 2 years' experience as a Registered Nurse
• Strong clinical assessment and critical thinking skills
• Excellent communication, organizational, and interpersonal skills
• Ability to work independently and as part of a healthcare team
• Proficient computer skills and experience with electronic health records
• Attention to detail and accuracy in documentation
• Availability Monday - Friday during normal business hours
