Job Description

Utilizing clinical expertise, the clinical mental health professional conducts psychosocial assessments, creates and implements appropriate care plans, evaluates crisis situations to determine the patient's needs and makes appropriate referrals, and administers family and individual treatment as needed. Together with patients' significant others, families, healthcare providers, insurance, local resources, and other stakeholders, this role collaborates. For the designated patient population, this role offers care in accordance with the established plan of care.

Requirements

• Recognizes and treats a range of emotional, behavioral, and mental health issues, adapting treatment plans to account for evolving patient needs.

• Directs patients seeking better well-being to various resources. Community services, treatment plans, support groups, psychiatrists, and other mental health professionals may be the source of these recommendations.

• Collaborates with the multidisciplinary team to create a discharge plan that outlines additional services, recommendations, and care to guarantee continuity of care. • Adheres to local, state, federal, and clinical program/facility requirements.

• Behaviors covered by the Mental Health Practice Act.

• Asses new, ongoing, and discharged patient requirements in area(s) of responsibility to prioritize and organize daily tasks.

• Provides patients and caregivers with social work consultation services. carries out psychological evaluations. Offers crisis bereavement, supportive, and further social work interventions.

• Educates, communicates, and coordinates with multiple patient care teams and outside agencies as necessary to ensure the patient receives quality care, and a timely discharge, and to identify and resolve delays and issues. Credentialing through medical staff office and peer review requirements may apply if working in the behavioral health setting.

• May provide support and debriefing, in conjunction with the Employee Assistance Program, for staff members following adverse events.

• Acts as an advocate on behalf of patients and families, where appropriate, to ensure effective communication with multidisciplinary care teams, outside agencies, and other resources as necessary.

• May identify patients at risk for potentially avoidable delays, extended stay, readmission, financial or complex discharge needs. Coordinate with care providers and leaders to reach optimal solutions.

Requirements

• A master’s level degree from a regionally accredited university.

• An active social work, counselor, marriage and family therapy, or psychologist license (LCSW, LMFT, LCMHC, PsyD, PhD, etc.)

• Two or more years of experience providing clinical psychotherapy to adults

• Experience and training in Cognitive Behavioral Therapy (CBT) and other evidence-based practices.

• Excellent writing skills for documentation of sessions.

• A clean record without any disciplinary action being taken against you by a licensing board in any jurisdiction.

Salary: $90k to $100k Annually.
