Tiburcio Vasquez Health Center is a non-profit community health center that is dedicated to promoting the health and well-being of our community by providing accessible, high-quality care by integrating primary care, dental care, WIC support, mental health counseling, community health education, and more.

The Role:

The Street Health Outreach program is a pioneering initiative dedicated to identifying and assisting individuals in Central Alameda County who are grappling with homelessness and face ongoing health and mental health challenges. Our Street Health Outreach Team is committed to empowering clients to achieve housing stability and enhance their overall well-being.

Utilizing a comprehensive, multidisciplinary approach, our program offers street- and venue-based outreach and engagement, assessments, essential health services, triage support, case management, and connections to housing agencies and mental health services. We collaborate closely with all participants to craft and implement action plans aimed at enhancing their health and wellness.

Under the guidance of the RN Care Manager, our Street Health Outreach Behavioral Health Case Manager works directly with participants, conducting behavioral health assessments, delivering brief interventions, and providing invaluable assistance in navigating mental health and substance use treatment systems. We approach this work from a harm reduction and trauma-informed perspective, ensuring the utmost care and support for our clients.

Compensation: $30.34$32.00 per hour, depending on experience

This is a full-time benefited position working 40 hours per week, typically Monday through Friday with periodic Saturday hours.

Summary of Responsibilities:

  • Conduct behavioral health assessments and offer brief interventions.
  • Triage and support clients in navigating mental health and substance use treatment systems.
  • Manage a caseload of clients requiring in-depth case management services.
  • Engage in outreach, provide health services, and collaborate with Housing Resource Centers.
  • Build lasting relationships through consistent visits with clients and potential clients.
  • Screen and identify individuals with medical and behavioral needs.
  • Prioritize individuals with high medical, behavioral health needs, or Substance Use Disorders for intensive visits aimed at improving health and ending homelessness.
  • Participate in regular meetings with Alameda County Healthcare for the Homeless.
  • Deliver culturally competent, trauma-informed, and harm reduction-based services.
  • Engage in regular outreach and build trust with individuals encountered at encampments and service locations according to a responsive schedule.
  • Respond to immediate survival needs as prioritized by clients, including basic needs, problem-solving, health education, and information about health and community services.
  • Perform assessments and administer appropriate screening tools (e.g., PHQ2 and PHQ9) and SBIRT services.
  • Provide crisis assistance and brief behavioral health interventions (Motivational Interviewing, Problem Solving Therapy, psychological/emotional support related to housing).
  • Conduct non-stigmatizing screenings to evaluate the needs of homeless individuals for mental health and support services.
  • Offer behavioral health services using brief treatment, problem-solving therapy, behavioral activation, and other Evidenced-Based Treatments, in a culturally, linguistically, and age-appropriate manner.
  • Provide ongoing consultation for team members.
  • Facilitate linkage and referrals to community services as needed, emphasizing an interdisciplinary approach.
  • Engage in multidisciplinary staff meetings and other staff processes to contribute to program development, staff development, program evaluation, and program operations review.
  • Maintain strict confidentiality regarding privileged administrative and patient information and complete required documentation promptly.
  • Complete Coordinated Entry Assessments for individuals seeking housing.

Requirements

    • High School Diploma is a mandatory requirement.
    • Obtain a valid Cardio Pulmonary Resuscitation (CPR) Card within three months of hire.
    • Possess a comprehensive knowledge of medical principles and allied services commensurate with training.
    • Demonstrate the ability to establish trust and respect with patients, staff, colleagues, external contacts, and fellow organizational members.
    • Proficiency in identifying potential escalating situations and safety risks, with the ability to de-escalate when necessary.
