Job Description

Behavioral Health Solutions is seeking a highly skilled and compassionate Case Manager to join our expanding team. As a leading provider of comprehensive behavioral health services, we specialize in offering state-of-the-art mental health treatment to individuals residing in Skilled Nursing Facilities and through our Outpatient Tele-Health program. At Behavioral Health Solutions, we are dedicated to improving the lives of our patients through personalized care and evidence-based interventions.

As a Case Manager in our Drug and Alcohol Program, you will play a crucial role in supporting individuals struggling with substance abuse issues. Working closely with our interdisciplinary team of mental health professionals, you will assess patients' needs, develop and implement individualized treatment plans, and provide ongoing monitoring and support throughout their recovery journey. Your empathetic and non-judgmental approach will create a safe and supportive environment for patients to explore their challenges and work towards lasting change.


Responsibilities
  • Conduct comprehensive assessments to evaluate patients' substance abuse history, mental health status, and treatment needs
  • Collaborate with patients and their families to develop individualized treatment plans based on identified goals and objectives
  • Facilitate group therapy sessions focusing on relapse prevention, coping skills, and substance abuse education
  • Monitor patients' progress and adjust treatment plans as necessary to ensure optimal outcomes
  • Coordinate care with other healthcare professionals, including psychiatrists, psychologists, and social workers, to provide a holistic approach to patient care
  • Collaborate with community organizations and external agencies to connect patients with additional resources and support services
  • Maintain accurate and up-to-date documentation of patient assessments, treatment plans, and progress notes

Requirements

  • Bachelor's degree in social work, psychology, counseling, or a related field (Master's degree preferred)
  • Minimum of 2 years of experience in a behavioral health setting, with a focus on substance abuse treatment
  • Strong knowledge of evidence-based practices in substance abuse treatment and recovery
  • Proficient in conducting comprehensive assessments and developing individualized treatment plans
  • Experience facilitating group therapy sessions focused on relapse prevention and coping skills
  • Excellent verbal and written communication skills, with the ability to effectively collaborate with interdisciplinary teams
  • Ability to maintain confidentiality and adhere to ethical guidelines and professional standards in providing patient care
  • Valid state driver's license and reliable transportation for local travel to various Skilled Nursing Facilities
