Job Description
We are seeking a Full Time Nurse Practitioner for a private Cardiology amp; Internal Medicine outpatient office in Brooklyn, NY.
We are willing to train. New grads welcomed.
Salary is $120K-$140K a year depending on experience plus full benefits.
Work Schedule:
·Your schedule will range from 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m.
·An average workweek totaling approximately 40 hours.
Responsibilities:
·Conduct comprehensive health assessments and screenings for individuals and families in the community.
-Provide Stress Tests
-Order Chest X-Rays and Diagnostics
-Treat Peripheral Arterial Disease (PAD)
-Manage patient comorbidities
-Treat and manage patients' hypertension, diabetes, high cholesterol
-Medication Management for patients with CHF and Atherosclerosis
·Diagnose and manage acute and chronic medical conditions, including preventive care and health promotion.
·Collaborate with community organizations and stakeholders to develop and implement health education programs.
·Provide culturally sensitive care and advocate for patients' rights and access to healthcare services.
·Coordinate referrals and follow-up care to ensure continuity of care for patients.
·Maintain detailed and accurate medical records in compliance with regulatory standards.
·Participate in community outreach events and health fairs to raise awareness and provide health screenings.
·Stay updated with community health trends, evidence-based practices, and relevant policies.
Requirements
Requirements:
·Current certification as a Nurse Practitioner; FNP, ANP, or AGNP.
·Active New York State APRN License
·Master's degree in nursing from an accredited program.
·Strong interpersonal and communication skills, with the ability to work effectively with diverse populations.
