Touchstone Health of San Antonio is actively seeking a qualified and compassionate Hospice Chaplain to provide spiritual comfort and bereavement support to hospice patients and families. The ideal candidate has spiritual and grief counseling experience, flexibility, and the ability to function autonomously. Duties amp; Responsibilites:

• Assessing potential bereavement needs of patients families
• Contributing to a resource list of community and church-sponsored grief support groups amp; activities
• Maintaining proper documentation of patient and family visits and bereavement services provided
• Assisting Bereavement Coordinator regarding pre-death bereavement assessments
• Provision of direct spiritual support to patients and families
• Educate fellow staff members to enhance awareness of the spiritual concerns of patients and families experiencing terminal illness and loss
• Maintaining proper documentation of patient and family visits
• Contributing to a resource group of clergy to whom specific aspects of spiritual care may be delegated
• Conducting and arranging for funeral and memorial services when necessary
• Collaborating with multi-disciplinary team to develop spiritual plan of care

Requirements

Qualifications amp; Requirements:

• Bereavement experience in varied multi-disciplinary settings
• Minimum two years experience in health or related field
• Valid Texas driver's license and insured, reliable transportation
• Demonstrating knowledge of grief process (normal/abnormal, anticipatory, bereavement)
• Awareness of community resources and agencies for referrals when appropriate
• Understanding of the CQI process and ensuring practices and actions reflect the CQI recommendations
• Ability to work effectively with a diverse ethnic and cultural patient population
• Bilingual preferred
