As a Primary Care Nurse Practitioner, you will play a crucial role in providing comprehensive primary care services to patients of all ages. You will be responsible for conducting physical exams, diagnosing and treating common illnesses, prescribing medications, and providing health education and counseling.

Job Responsibilities:
- Act as a key resource in providing clinical and operational guidance and support
- Deliver advanced nursing clinical care for patients by regulations and national standards
- Collaborate with physicians and develop clinical protocols
- Provide comprehensive advanced nursing care, including assessments, interventions, and patient education
- Collaborate with patients, families, and other team members for assessment and care planning
- Ensure compliance with quality of care measurements and standards
- Assess, educate, and improve patient and caregiver knowledge of chronic disease management
- Communicate with internal and external care partners for timely interventions
- Manage the quality of medical record documentation and billing information
Requirements
- License and current registration to practice as a Registered Professional Nurse in New York State
- License and current registration to practice as a Nurse Practitioner in the State of New York, with a specialty in adult health, family health, or gerontology.
- 5+ Years of overall clinical experience
- 2+ Years of experience as a Nurse Practitioner utilizing a full scope of practice
- Clinical home care experience or 2+ years of managerial experience preferred
- NYS Valid driver's license, as determined by operational/regional needs may be required
- Maintains credentialed status with client's Medical Care at Home and associated managed care plans
- Maintains NPI, Medicaid, and Medicare provider numbers
- Certified by ANCC or another accrediting Nurse Practitioner body to bill Medicare and meet credentialing requirements
- Master's Degree of Science in Nursing or another graduate degree from a nurse practitioner educational program registered by the New York State Education Department as qualifying for NP certification (licensure)
- Current ANCC or AANP certification as an adult, family, or geriatric nurse practitioner
- Demonstrated knowledge of HEDIS and QARR quality measures, ICD-9, and CPT coding for reimbursement of services required
- Bilingual skills, as determined by operational needs, are preferred
- Monday - Friday 8:30 am - 4:30 pm
- Roughly 15 in-home visits per week
