Eminence Home Care in Hartford, CT is looking for a Nurse Manager to join our team.

The Nurse Manager will oversee all aspects of nursing care for our homebound patients, ensuring that everyone receives the highest quality care possible. The ideal candidate will be a skilled nursing professional with a strong background in home health care, has an understanding of state and federal Medicare regulations and a passion for helping patients live their best lives.

What you will be doing:

• Develop, implement, and evaluate nursing care plans for homebound patients.
• Supervise and evaluate the performance of all nursing staff and ensure compliance with home health standards.
• Ensure that all nursing staff are properly trained and certified in their areas of expertise.
• Oversee and support a home health therapy team.
• Collaborate with other healthcare professionals to ensure optimal patient care outcomes.
• Develop, implement, and maintain policies and procedures for nursing care.
• Maintain accurate patient records and ensure compliance with all regulatory requirements.
• Oversee all OASIS assessments, educate and train as needed around coding.
• Oversee insurance authorization process.
• Monitor and continually strive to improve CMS star rating.
• Provide nursing care to patients as needed.

What we expect from you:

• Bachelor's degree in nursing or related field required; master’s degree preferred not required.
• Registered Nurse (RN) licensure in the state of Connecticut
• Minimum of 3 years of experience in home health care (preferred)
• Strong leadership and management skills
• Ability to develop and implement effective nursing care plans.
• Excellent communication and interpersonal skills
• Experience working with multi-disciplinary home health teams.
• Familiarity with electronic medical records (EMR) and other healthcare technology
• Experience with OASIS in home assessments.
• Familiarity with Medicare and Medicaid regulations, and the related survey process locally
