Seasoned Recruitment is currently seeking a dedicated and compassionate Medical Social Worker to join our team in Illinois. In this role, you will provide essential support and assistance to patients and their families by addressing their psychosocial needs and helping them navigate through the healthcare system. • Assess patients' psychosocial needs and develop appropriate care plans
• Provide counseling and support to patients and their families
• Coordinate discharge planning and arrange for appropriate community resources and services
• Collaborate with healthcare professionals to ensure a comprehensive approach to patient care
• Advocate for patients' rights and promote their overall well-being

Requirements

• Master’s in Social Work from a graduate school accredited by the Council of Social Work.
• One (1) to two (2) years of post-graduate social work experience in a health care setting.
• Must hold, in good standing, a license as a Medical Social Worker issued by the state in which you work.
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
• A commitment to consistently meet critical deadlines for charting
• The skills needed to self-manage your time and schedule
• Demonstrated experience with tablets, mobile phones, and EMR software
