Job Description
Seeking a Licensed Practical Nurse in the Jefferson City, MO and surrounding areas to join our Hospice Team!
Our Hospice LPN's help to meet the physical, spiritual and emotional needs of our clients and their families. Our patients are facing end-of-life healthcare needs, and we are looking for an LPN that can provide the needed nursing care with dignity, compassion, and a warm heart.
Shift Availability: Full Time, Days
Pay Range: $50,000 - $70,000

Responsibilities
- Seeing Patients every two weeks and then weekly as health declines
- Building trackers and building referrals of patients
- Completing notes on each visit
- Some office days required to complete paperwork and uploading
Requirements
- Missouri or compact nursing license
- Valid Driver's License
- Monitor and record patient's condition and document provided care services via electronic charting
- Problem solving skills and ability to multi-task
- Excellent Communication Skills
- Positive, Professional attitude
