Job Description

Seasoned Recruitment is currently seeking a highly skilled and dedicated Integrated Pharmacist for a position based in Houston, Texas. As an Integrated Pharmacist, you will play a critical role in providing comprehensive pharmaceutical care to patients in an integrated healthcare setting. You will work closely with healthcare providers to optimize medication therapy, improve patient outcomes, and enhance the overall quality of care.

Responsibilities:

  • Conduct medication therapy management (MTM) and comprehensive medication reviews.
  • Collaborate with healthcare providers to develop and implement patient-centered care plans.
  • Perform medication reconciliation and identify drug-related problems.
  • Provide medication counseling and education to patients on proper medication use.
  • Monitor patient responses to therapy and recommend adjustments as needed.
  • Document interventions and outcomes in patient records.
  • Participate in interdisciplinary team meetings and care conferences.
  • Contribute to quality improvement initiatives to enhance medication safety and effectiveness.
  • Stay updated on new medications, evidence-based guidelines, and emerging healthcare trends.
  • Adhere to all regulatory and legal requirements related to pharmacy practice.

Requirements

  • Doctor of Pharmacy (PharmD) degree from an accredited institution.
  • Pharmacy licensure in the state of Texas.
  • Certification in Medication Therapy Management (MTM) or willingness to obtain.
  • Minimum of 3 years of experience as a clinical pharmacist or in an integrated healthcare setting.
  • Strong clinical knowledge and proficiency in medication therapy management.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a collaborative healthcare team.
  • Attention to detail and strong problem-solving abilities.
  • Proficiency in using electronic health record (EHR) systems.
  • Flexibility to work variable shifts and weekends as required.
