This clinical training program is a paid internship which aims to bridge the skill gaps of new NP/PA graduates to enable smooth transition to a NP/PA role. This program will help build proficiency and efficiency demanded in employment settings and provide an opportunity for new grads to understand our work culture, patient population.
This is a full-time benefited position working 40 hours per week, typically Monday through Friday with periodic Saturday hours.
Tiburcio Vasquez Health Center is a non-profit community health center that is dedicated to promoting the health and well-being of our community by providing accessible, high quality care by integrating primary care, dental care, WIC support, mental health counseling, community health education and more.
Compensation: $120,000 annualized during the program, and $136,052.80 upon completion.
Duration of the program ranges, depending on the provided, up to 6 months which is divided into 3 Tiers:
First Tier:
- Administration of medications and immunizations to patients
- Health instruction/ Education to patients
- Medication reconciliation
- Referrals to community health resources
- Triage patients, comprehensive history taking, examination and documentation
- Supervise clinic workflow and assist providers and MA when needed.
- Organize orientation, training, growth, and development of LVN and MA staff
- Maintain inventory of medical supplies and reorder as necessary
- Assist providers with tasks delegated
- Attend general and medical staff meetings and participate in quality improvement projects.
- Assist the Chief Medical Officer and COO on matters relating to nursing practices and participates in the development of policies and protocols for patient care.
- Licensed and credentialed to practice as an NP provider.
Second Tier:
- Inbox coverage
- Managing low acuity patients; UTI, Immunizations, Depo Shots, Medication refills, Sore throat, Gastroenteritis, high BP, asthma exacerbation, vaginitis, joint pains, annual physicals
- Patient education on DM, tobacco cessation, obesity
- Anxiety and depression management and linking to BH
- Seeing new patients to TVHC
- Placing referrals for acute ER visits (ortho/ophthalmology)
- Shadow/ Precept and scribe to grasp EPIC workflow. day per week with Preceptor
- Panel building starts
Third Tier:
- Continue nursing duties and previous quarter expectations
- Manage chronic conditions like COPD, CHF, CKD, DM, and Thyroid disorders
- To be able to manage a complex patient with multiple conditions
- To be able to manage time with increasing panel size, the goal of 18 patients at the end of this tier
- Able to use cost-effective and evidence-based diagnostics and referrals.
- Timely documentation in EPIC of patient visits, management of inbox, and paperwork completion
- Referrals to specialists as needed for complex and high-priority patient cases.
- Adheres to the ethics of the organization and provides compassionate care to patients.
- Develop a strong knowledge base.

Requirements
- Must be in possession of a current medical license to practice in the State of California.
- Current CPR certification.
- Must obtain DEA license prior to starting or within the first 2 months of the program.
- Board Certified in designated specialty field of practice is required.
- Previous experience providing medical care within a community health center environment preferred.
Qualifications:
- Have a thorough knowledge of the principles and practices of medicine and allied services commensurate with his/her training.
- Demonstrated ability to build the trust and respect of patients, staff, colleagues, external contacts, and other organizational members.
- Willingness and ability to work periodic evenings and/or weekends.
- Bilingual, especially Spanish, is preferred but not required.
- Excellent problem-solving skills required, including creativity, resourcefulness, timeliness, and technical knowledge related to analyzing and resolving medical problems.
- Sensitivity to needs of culturally and linguistically diverse patient and employee population.
- Demonstrated ability to utilize computer technology and to willingness to develop and adapt to the evolving technological requirements of modern medical health center practices required.
