Job Description

The Case Manager provides quality case management and psychotherapeutic services to all patient and their families. Serves as a member of interdisciplinary team supporting the organization's treatment program and philosophy and assures the deliverance of quality treatment to patients and their families. Must be able to work weekends

KEY RESPONSIBILITIES:

  • Develops and coordinates an individualized discharge plan for the patient by utilizing treatment team and written chart information to determine the patient's aftercare needs.
  • Accesses by phone or written correspondence family or community resources such as nursing homes, group homes, or private mental health practitioners to meet patients' aftercare needs.
  • Master's level Case Manager provides individual, group, and family psychoeducation-oriented care using various professional treatment modalities.
  • Coordinates individualized discharge planning for patients with physician.
  • Documents communication in progress notes regarding patient in medical record.
  • Attends treatment team meetings with multidisciplinary team.
  • Reports incidents of suspected abuse using prescribed forms and protocols from the relevant community/state agency.
  • Maintains effective working relationships and communications with internal and external customers.
  • Schedules, attends, and documents court hearings as required.

Requirements

  • Bachelor’s Degree in Psychology or related field required; Master's degree preferred.
  • Minimum three (3) years behavioral health experience or combination of education and experience preferred.
  • Familiarity with medical and psychological terminology and human behavior principles
  • Experience with therapeutic limit-setting or an understanding of level systems or other behavior management modalities helpful
  • Ability to maintain professional boundaries
  • Maintains confidentiality of patients at all times
