Job Summary:
The Licensed Physical Therapist (PT) will provide safe and effective intermittent home-based physical therapy visits to home health clients per the physician/health care provider’s orders and as defined by the Minnesota Board of Physical Therapy Practice Act. The evaluation and/or treatment of any person by the employment of physical measures, use of therapeutic exercises. rehabilitative procedures, with or without assistive devices will be for the purpose of preventing, correcting, or alleviating a physical or mental disability. Physical therapy includes evaluation other than medical diagnosis, treatment planning, treatment, documentation, performance of appropriate tests and measurement, interpretation of orders or referrals, instruction, consultative services, and supervision of supportive personnel. The Licensed Physical Therapist (PT) practices with compassion, accountability, and proficiency. The PT performs all functions in accordance with licensing standards, agency policies, payer regulations, and other state, federal, and local applicable laws.
Job duties also include but are not limited to: conducting OASIS assessments, performing visit documentation, obtaining physician orders, communicating with health care professionals, instructions to physical therapy assistants and home health aides. The physical therapist reports to the Home Health Clinical Manager.
Essential Responsibilities:
- Provides physical therapies to agency clients as directed by the health care provider, per the established plan of care, licensing regulations, within the scope of practice, and agency policies and procedures.
- Conducts a focused assessment of the health status of an individual client through the collection and comparison of data to normal findings and the individual client’s current health status,
- Reports changes and responses to interventions in an ongoing manner to a registered nurse and/or the appropriate licensed health care provider.;
- Participates with other health care providers in the development, modification, and coordination of a plan of care;
- Communicates information on a timely basis, consistent with agency policy and urgency of the situation. Documents all communications in the clinical record.
- Determines and implements appropriate interventions within the plan of care;
- Assigns physical therapy tasks to other physical therapy assistant(s) and instructs home health aides on on-going tasks;
- Conduct the evaluation of responses to therapy interventions;
- Collaborates and communicates with other health care providers;
- Provides instruction and education on treatments to individual clients and family caregivers;
- Provides input into the development of agency policies and procedures; and
- Is accountable for the quality of care delivered, recognizing the limits of knowledge and experience and addressing situations beyond the nurse's competency.
Requirements
- Bachelors Degree or higher in Physical Therapy from an accredited educational institution.
- Active Physical Therapist license in the State of Minnesota with a minimum of 6-12 months clinical physical therapy experience.
- A minimum of 6 months experience providing physical therapy services
- A minimum of 6 months of physical therapy experience in a Medicare certified home health agency.
- CPR certified preferred.
- Driver’s license and reliable transportation for home visits
- Effective written and verbal communication skills
- Crisis Intervention skills
- Ability to understand and evaluate the potential for medical or behavioral emergencies
- Knowledge of the needs of adults with chronic medical and mental health conditions
- Knowledge of the Vulnerable Adult Act and reporting process
- Approved criminal background check
