Job Description

We are seeking an experienced and caring RN to provide care for residents in the Health amp; Rehab Center. We are looking to fill a Part-Time Plus (24-35 hours/week) 7:00 am to 3:30 pm position, required to work a minimum of every other weekend. Ideal candidates will have flexible availability to work other shifts, weekends, and holidays as needed. The Registered Nurse - (RN) reports to the Unit Manager and assumes the charge position on assigned shifts. The RN is responsible for the nursing care of the residents in the Health and Rehabilitation Center (HRC) and for any emergency care of residents and/or staffing problems that may arise.


Responsibilities
  • Rounds on assigned residents at least twice per shift to ensure resident safety and implementation of the plan of care; makes rounds with outgoing and incoming nurse to discuss residents on caseload and observed changes
  • Supervises CNA activities to ensure resident needs are met; obtains information from CNAs regarding response to care, changes in condition, or other information gathered during the shift; holds CNAs accountable for timely and accurate completion of scheduled vital signs and weights
  • Performs professional nursing tasks and nursing procedures including venipuncture and intravenous therapy according to current standards, policies, and procedures
  • Appropriately addresses allegations or sightings of potential abuse/neglect by rescuing the resident, reporting it immediately and reassuring the resident
  • Monitors staff for professionalism and positive interaction with residents and other Williamsburg Landing employees
  • Assists with resolving staffing needs and emergency staffing problems as they arise
  • Takes emergency calls for all of Williamsburg Landing and visits residents as required
  • Orders labs and medications per the physician's order
  • Keeps work environment clean and tidy
  • Ensures that infection control protocols are followed by fellow team members
  • Contacts physicians, other professional personnel and families, as necessary
  • Takes house calls, if requested, for Independent Living residents; drives company vehicle to respond to the house calls as requested by public safety
  • Documents resident observations, changes in condition, response to care, delivery of medication and treatments, and other pertinent information according to departmental standards, policies and procedures
  • Maintains and updates care plans as necessary
  • Utilizes the Interact SBAR tool for change in resident conditions
  • Informs the Director of Nursing of any problems that arise during the shift
  • Complies with and enforces department and company policies and procedures including standards of conduct, attendance, dress, and safety
  • Greets new admissions within 30 minutes of their arrival and thoroughly completes the admission process
  • Comprehensively completes daily skilled notes
  • Serves on committees as requested
  • Ensures staff have adequate supplies and equipment to carry out their responsibilities; ensures supplies are being managed appropriately through the Orbits system
  • Ensures staff complete all shift responsibilities before the end of the shift
  • Serves as a positive mentor for new team members and provides a comprehensive onboarding experience
  • Assists with performance evaluations for CNAs and staff in-service training
  • Assists management staff during licensing reviews
  • Maintains positive and professional relationships with residents, guests, and other Williamsburg Landing employees
  • Answers the facility telephone promptly and if unable, delegates the task to another team member
  • Participates in facility performance improvement initiatives
  • Performs other duties as assigned

Requirements

  • Registered Nurse in the Commonwealth of Virginia.
  • Strong verbal and written communication skills.
  • 2 years of nursing experience in an acute care or long-term care setting.
  • Ability to obtain the Dementia Practitioner Certification within 1 year of employment.
  • Comprehensive knowledge of nursing procedures, theory and practice with an emphasis on current trends in gerontological nursing in a long term or acute care setting; ability to interact well with physicians and other health care professionals; ability to exercise sound professional judgment and respond appropriately in an emergency; ability to lift and carry a minimum of 50 pounds; ability to stand and walk for entire shift and safely lift and transfer residents.
