The Registered Nurse Case Manager (RNCM) functions as a member of the Interdisciplinary Team (IDT) in order to provide routine and emergency assessment of patients, education to patients and caregivers, and care coordination to ensure the needs of patients and their families are met.
He/she helps the patient and family maintain their maximum level of comfort and coping. He/she works in a flexible schedule to meet agency staffing needs and provides after-hours emergency support to the patient and family (on-call responsibilities are part of the role requirement). He/she applies the rules and regulations of state and federal regulatory agencies and other certified agencies.
Requirements
Registered Nurse licensed in Pennsylvania
1+ years of Nursing practice experience
Hospice, Home Health, and/or Long-term Care experience is HIGHLY preferred
