Specific Responsibilities of the Job:
- Upon admission into Anchor, the case manager will meet with the client within 5 days of admission to start working on the continuing care plan. The case managers primary goal is to make sure all clients are leaving with all community resources to include 12 step meetings, further SUD treatment, mental health, MAT providers, care coordination, housing, DSS/food stamps, recovery coaching, care coordination, legal resources, medical, medication management and transportation.
- Provides individual and group education/treatment services in accordance with licensing, certification, amp; productivity standards. In some cases the case manager would be assigned a client and would need to meet with the client for individual, treatment planning and discharge paperwork.
- Attends and actively participates in trainings and weekly team meetings following established procedures for case assignment and review.
- Provides internal amp; external outreach services to include but not limited to staffing coverage, crisis intervention, and training as scheduled amp; needed.
- Cooperates with work team/agency on accomplishment of strategic objectives and activities as needed.
- Assists clients with obtaining insurance and provides insurance authorizations for private insurance clients.
- Adheres to all policies, procedures amp; regulations.
- Maintains a positive work atmosphere that promotes Walden’s culture, such as but not limited to, responding appropriately to the cultural differences of staff, clients, residents, amp; referral sources.
- Communicates with Anchor counselors, admissions coordinator, and medical staff any pertinent information regarding client’s needs.
- Communicates with manager any patterns in noticing challenges, barriers, and things that are going well.
- Other duties as assigned.

Requirements
Licensure, Education, amp; Experience:
- Bachelor’s degree in related field required; Master’s degree in related field preferred.
- Trainee status with the MD Board of Professional Counselors amp; Therapists, CSC-AD, CAC-AD or higher certification and/or active Maryland LGPC, LGSW, LCPC or LCSW-C licensure preferred.
- Two years previous experience in case management required.
