Job Description

The purpose of this position is to provide general nursing care and primary care team leadership, working collaboratively with physicians and multidisciplinary team members. The Care Team Nurse will promote and restore patient’s health as they provide, delegate, evaluate and coordinate nursing care for all patients. This individual may oversee and participate in clinical care and case management of patients.


Requirements

Essential Functions:

  • Assess, plan, direct and evaluate total nursing care as determined by the patient’s age related physical, psychosocial, and cultural needs in accordance with established standards, policies and procedures of the health center.
  • Promotes patient's independence by establishing patient care goals.
  • Assists provider with examination and procedures as requested.
  • Provides case management for complex, high needs patients by serving as a patient advocate, patient resource and contact person for patient treatment coordination.
  • May provide health related counseling on the prevention of communicable diseases; coordinate health programs with outside organizations and agencies; introduce general health related information and instruction to individuals, families and community groups.
  • Assess and evaluate patient’s response to treatment and medication.
  • Keep accurate patient records; maintain confidentiality at all times.
  • Properly prepare and administer immunizations and vaccines.
  • Assist in research and investigation of communicable diseases when requested.
  • Coordinate referrals to IHSS.
  • Serves as a resource person to other staff in the area of one’s own clinical expertise.
  • Compliance with all state and federal laws and regulations, as they pertain to position including; HIPAA, sexual harassment, scope of practice, OSHA, etc.

Minimum Qualifications:

  • Valid California RN, LPN, or LVN License.
  • Experience with an electronic health record system.
  • Ability to chart visits comprehensively.
  • Computer literacy including word processing, office software, patient processing, and electronic mail systems.
  • 2 years of experience working in community health as a member of a multi-disciplinary team preferred.
  • Care and compassion in working with low income and indigent populations; Skill in working with people of varied social, ethnic and economic backgrounds.
