Outreach Recovery is seeking a Physician's Assistant/PsychiatricNurse Practitioner to join our Mental Health and Addiction Team in the Philadelphia, PA area. This position will task the provider with seeing patients in a medically-assisted recovery program. Working with our Addiction Team the Provider delivers medical care for specific patients, including formulating a diagnosis and a treatment plan and providing continued follow up patient care.
- Provide direct primary and acute addiction care services to adult patients
- Perform initial and on-going assessment of patients' medical, physical and psychosocial status
- Obtain and evaluate health and medical history; perform a physical exam, conduct preventive screening procedures based on age and history
- Identify medical and health risks
- Complete accurate and timely documentation of observations, interventions, plan of care, referrals and follow-up
- Formulate appropriate differential diagnosis based on history, physical examination and diagnostic testing
- Identify needs of the individual, family or community and develop a plan of care and/or education plan
- Make appropriate referrals to other health professionals and community agencies
- Reassess and modify the plan as necessary to achieve medical and health goals
- Contribute to continuing education, expert blogs, and ongoing outreach marketing activities
- Develop a patient education plan relevant to the condition and patient/ family needs
- Participate in performance improvement activities as assigned

Compensation: $75 per hour
Requirements
- Master's Degree in Nursing; valid Pennsylvania APN license (Advanced Practice / Nurse Practitioner License); CNS (Clinical Nurse Specialist) in adult psychiatry / mental health with substance abuse experience; computer proficiency. Wellness and Recovery orientation.
- Valid Drug Enforcement Administration (DEA) Certificate listed in Pennsylvania
- DATA 2000 waiver 100 level required
