Job Description

Under the guidance of the Co-Program Director, provides quality individual and group counseling and harm reduction education to SACHR participants. Assure that intervention fidelity is maintained. Job duties include, but not limited to:

  • Implement the Harm Reduction Behavioral Health Initiative at SACHR.
  • Provide Individual amp; Group behavioral health interventions (including intake, psychosocial [re]assessments) to participants of all departments, i.e., Harm Reduction/Syringe Access Program, Women’s Services Program, and Ryan White Program.
  • Assist participants with external referrals as needed.
  • Provide clinical support to the Ryan White Part A - Supportive Counseling amp; Family Stabilization Program staff.
  • Uphold SACHR’s policies and procedures, including the policies and procedures set forth by the NYS Department of Health and the NYC Department of Health amp; Mental Hygiene regarding behavioral health interventions including HIPAA requirements.
  • Complete monthly chart reviews ensuring all documentation is thorough and accurate.
  • Perform data entry relative to behavioral health interventions in the Electronic Health Record, AIRS, TREAT, etc.
  • Organize bi-weekly (and as needed) participant case conferences.
  • Train staff in basic behavioral health skills.
  • Participate in supervision, staff meetings, and case conferences as required.
  • Represent SACHR at community, professional and/or coalition meetings.
  • Perform other duties as assigned.
  • Participate in regular supervision, training, and staff meetings.

Requirements

  • Licensed Master of Social Work or Licensed Mental Health Counselor preferred; Master in Social Work required.
  • At least two years’ experience providing behavioral health services to vulnerable populations; knowledge of substance use, mental health, HIV, Hepatitis, etc.
  • Knowledge of harm reduction model.
  • Excellent communication skills.
  • Good computer / technical skills.
  • Bilingual English/Spanish preferred.
  • Flexible schedule.
