Job Description

Under the supervision of a Programs Manager, the Services Coordinator (SC) will provide service coordination for the Clients programs to include Supportive Services for Veteran Families, (SSVF) and the Rapid Rehousing Programs delivered throughout the County. Services Coordinators are expected to adhere to the highest standard of social service ethics in the performance of their job requirements. The SC will work in partnership and collaboration with multiple entities to assist in rapidly rehousing and/or achieving stabilization for homeless individuals and/or their families.

ESSENTIAL FUNCTIONS: (Depending on the area of assignment, duties may include, but are not limited to the following :)

  1. Verify eligibility of referred homeless individual and/or family to participate in THE CLIENTS’ program(s).
  1. Conduct an initial intake and assessment (s) with customers once they are identified as eligible program participants. Assess families for strengths and barriers, including employment, mental health, education, etc., and coordinate the appropriate services.
  1. Develop and monitor Individualized Service Plan (ISP) based on the assessment, by completing appropriate forms with customers to identify specific actions and/or resources needed for successful stabilization of the customers.
  1. Provide THE CLIENTS Housing Specialist and/or partners with customers’ immediate housing needs and provide reasonable support to result in placement of customer in safe and adequate housing. Includes assisting with collecting documentation and/or coordinating transportation, as needed.
  1. Ensure that THE CLIENTS’ programs will offer time-limited, individualized financial assistance designed to assist customers obtain and retain permanent housing. Financial assistance is to be provided at a level that enables the customer to maintain housing while they may seek to: increase income; learn to manage a household budget; relocate to affordable housing; reduce expenses; and/or overcome other barriers in order to sustain their housing.
  1. Coordinate and refer services with other social service agencies to provide mental health, medical, dental care, and/or rehabilitation services. Link customer to services in the community for mental health treatment, and physical health as needed. Leverage resources available within the community including faith-based organizations as appropriate to the situation in order to benefit the customer.
  1. Make referrals to the County of San Bernardino Department of Behavioral Health (DBH) for Life Skills Training, mental health services, substance abuse services, or other needs as appropriate.
  1. Utilize the Housing First philosophy to ensure that treatment for mental health, substance abuse needs, or resolution of other barriers is not a condition of services or program participation. This is to include coordinating services in crisis situations.
  1. Schedule and conduct meetings with customers on a regular basis. This requires flexibility of meeting regularly in locations that may include administrative offices, site-based service facilities, in the community, and/or customer residence, depending on customer capability.
  1. Effectively educate participants about budgeting, financial management, including support with filing tax returns, filing for Earned Income Tax Credit, banking and credit repair. This may include training customers in functional daily living skills in order to maintain housing.
  1. Record service coordination efforts and outcomes; create and maintain exemplary customer files in accordance with THE CLIENTS guidelines; complete weekly activity reports and other required documentation in a timely manner.
  1. Regularly report customer progress to Supervisor and others as requested. This includes but is not limited to completing assessments, Individualized Service Plans, Service Transactions, and Exit Forms as necessary for program or organizational requirements. Actively participate in service coordination reviews and conferences.
  1. Enter data about the homeless individuals and families seeking services into the Homeless Management Information System (HMIS) and YARDI for all data tracking information in a timely manner.

  • Knowledge in working with the homeless population, homeless households, homeless services, and prior housing work experience

  • Demonstrated ability to work independently and effectively on a team
  • Computer skil
