Job Description

Seeking a compassionate, enthusiastic, goal orientated, and resourceful LPN to manage the after hours care of our patients and families. The On-Call Team responds to after hours phone calls promptly and appropriately, and makes patient visits to ensure the comfort and safety of clients and their caregivers.

  • Full Time On Call position - 7 days on, 7 days off
  • Hospice Experience Preferred
  • $2,000 Sign on Bonus!!!

Responsibilities

  • Provide education to the patients/family members regarding the end of life process, plan of care and offers guidance with health care decisions.
  • Consult with the attending physician and Hospice Medical Director concerning changes in patient condition requiring order changes and/or changes to plan of care.
  • Coordinate hospice care by working with other team members as well as the patients/family members to ensure needs are met.
  • Conducts initial as well as on-going comprehensive assessments.
  • Preform evaluations for hospice care along with informational visits.

Requirements

  • Demonstration of strong communication skills
  • Ability to work in fast paced environments
  • Kansas Nursing License
  • Strong organizational and self management skills
  • Strong and compassionate customer service skills
  • Valid driver's license
