POSITION SUMMARY:The Per Diem LISCW will work with patients in our Gender Affirming Hormone Therapy Program to provide surgical support letters for gender affirmation surgery.ESSENTIAL RESPONSIBILITIES
  • Maintains current LICSW licensure
  • Conducts patient assessments and writes surgical support letters for gender affirmation surgeries within two weeks of obtaining a referral
  • Documents all patient encounters completely and accurately in patient’s electronic medical record
  • Reviews amp; interprets the patient’s history as documented in the medical record
  • Uses of Teladoc or other platform as designated by PPLM to conduct virtual consultations with patients
  • Collects, tracks, and reports required program data including key performance indicators as required
  • Maintains patient confidentiality; is comfortable handling sensitive personal information.
  • Reviews amp; interprets the patient’s history as documented in the medical record
  • Reads, understands, and uses all relevant PPLM protocols, policies, and procedures; Keeps updated on all changes and incorporates those changes into practice
  • Other tasks and responsibilities as needed.

QUALIFICATIONS:
  • Master’s degree in social work and LICSW required
  • Must have some availability during normal business hours to conduct patient assessments
  • Strong interpersonal skills and ability to communicate information effectively
  • Ability to relate to individuals of varying backgrounds and age in a non-judgmental yet empathetic manner
  • Prior experience working in a health care setting a plus
  • Computer experience, especially Microsoft Office Suite, required.
  • Familiar with documenting in electronic health record. Experience with EPIC and Teladoc a plus.
  • Excellent listening skills.
  • Good organizational skills, ability to work independently.
  • Demonstrated critical thinking
  • Demonstrated ability to work with people of different races, ethnicities, sexual and gender identities and expressions, ages, abilities, and experiences
  • Commitment to addressing the impact of racism and other oppressions on sexual health
